Here’s how it works: SD Medical Cards and Wellness requires medical records stating that you have a current chronic diagnosis, debilitating disease, a medical condition or its treatment that produces one or more of the following:
And positive status for Human Immunodeficiency Virus (HIV)
Associated with severe or chronic pain, nausea or severe vomiting, or cachexia or severe wasting
Including those characteristic of multiple sclerosis.
If you have one of the qualifying conditions or symptoms, please schedule an appointment to meet our medical provider, Dr. Zachariah Jensen. A determination will be made if you are eligible to purchase a medical card certification.
Dr. Zachariah Jensen grew up in Central California and completed his undergraduate education at the US Air Force Academy in Colorado Springs, CO. Following graduation, he worked as a transportation and logistics officer in multiple states and deployed several times. Dr Jensen left full-time Air Force employment, returned to school, and graduated from medical school in the first class of Liberty University College of Osteopathic Medicine (LUOCM) in Lynchburg, Virginia. He completed his intern year in Physical Medicine and Rehabilitation at the University of Kentucky in Lexington, Ky. During this year he was recruited back to LUCOM where he graduated as the inaugural resident from their first residency program as a specialist in Osteopathic Neuromusculoskeletal Medicine. Wellness is a passion for Dr Jensen, and he believes that there is a better way to provide care that begins with having a relationship with the patient. Outside of work he loves outdoor activities, hunting, vehicles, and fitness.
Once your application is approved, your medical card will be mailed to you and you will then be able to purchase medical cannabis from a certified dispensary!
